Introduction & purpose: Smoking is one of the most important health threats among students, and psychological and social factors play a fundamental role in its occurrence. The present study aimed to develop a structural model of the effect of social support and self-esteem on smoking rates with the mediating role of mental health in medical students.
Methods: This descriptive-correlational study was conducted in the form of structural equation modeling. The research population included 4000 medical students at the Urmia Medical Sciences Branch of Pardis University in the second semester of the academic year 2023-2024, from whom 351 were selected by simple random sampling. The data collection tools included the Ziman et al. (1988) Perceived Social Support Questionnaire, Rosenberg's Self-Esteem (1965), Hill et al.'s Smoking Propensity (1997), and Keys' Mental Health (2002). Data analysis was performed with SPSS-26 and Smart PLS-3 software. Results: Self-esteem (β=0.454) and social support (β=0.479) had a positive and significant effect on mental health. Also, social support (β=-0.475) and mental health (β=-0.133) showed a negative and significant effect on smoking rate, and the direct effect of self-esteem on smoking was not significant. The indirect effects through mental health were significant and the model fit was strong (GOF=0.578).
Conclusion: Strengthening social support and promoting mental health can be effective in reducing cigarette smoking among students, and mental health plays an important mediating role in these relationships.
