Abstract
Introduction & Purpose:  Type 2 diabetes is one of the most common chronic diseases, which, in addition to physical complications, also has psychological impacts. The aim of this study was to compare the effectiveness of schema therapy based on mindset, with and without self-compassion training, on emotional schemas, self-perception, and glycated hemoglobin (HbA1C) in patients with type 2 diabetes.
Methods: This semi-experimental study with a pre-test–post-test design was conducted in the second half of 2024. The statistical population included individuals with type 2 diabetes in Mashhad. Thirty-six eligible participants were randomly assigned to two experimental groups and one control group. The therapeutic interventions consisted of 10 weekly sessions based on the schema therapy protocol and self-compassion training. Research instruments included the Leahy Emotional Schemas Questionnaire, Shusterman Self-Perception Questionnaire, Goldberg and Hiller General Health Questionnaire, and HbA1C tests to assess emotional schemas, self-perception, mental health, and blood sugar control. Data were analyzed using multivariate analysis of covariance (MANCOVA) and Bonferroni post hoc tests in SPSS version 26, with a significance level of 0.05.
Results: The mean(SD) age of participants was 33.61 ( 6.9) years, and the gender composition was 75% female and 25% male. Results showed that both interventions significantly improved emotional schemas (F = 8.39, η² = 0.11, P < 0.05), enhanced self-perception (F = 6.03, η² = 0.13, P < 0.05), and reduced blood sugar levels (F = 7.30, η² = 0.15, P < 0.05). Furthermore, results indicated that the schema therapy combined with self-compassion training produced greater improvement than schema therapy alone, due to higher mean differences (P < 0.05).
Conclusion: Self-compassion training can enhance the effectiveness of schema therapy in improving the psychological and physical condition of patients with type 2 diabetes.
