fa ارزیابی اجرای برنامه ی پزشک خانواده از دیدگاه مدیران و کارشناسان اجرایی بر اساس مدل دونابدیان: مطالعه ای مقطعی در سال 1401 Evaluation of the Implementation of the Family Physician Program from the Point of View of Managers and Executive Experts based on the Donabedian Model; A Cross-sectional Study in 2022 مدیریت خدمات بهداشتی و درمانی Management of Health Services پژوهشي-کامل Research(Original) مقدمه و هدف: برنامه‌ی پزشک خانواده با اهداف ایجاد نظام ارجاع در کشور، افزایش پاسخ‌گویی در بازار سلامت، افزایش دسترسی مردم به خدمات سلامت، کاهش هزینه‌های غیرضروری در بازار سلامت و افزایش پوشش خدمات اجرا می‌شود. این مطالعه با هدف ارزیابی میزان اجرای برنامه‌ی پزشک خانواده از دیدگاه مدیران و کارشناسان ستادی با رویکرد فرایندی در سال 1401 انجام شد. روش کار: در یک مطالعه‌ی توصیفی‌تحلیلی، 231 نفر از رؤسای مراکز بهداشت شهرستان و معاونان، مسئولان واحدهای فنی و اداری (مدیران) و کارشناسان واحدهای فنی و اداری (کارشناسان ستادی) در غرب استان گلستان به روش سرشماری شرکت کردند. ابزار جمع‌آوری داده‌ها، پرسش‌نامه تطبیق‌یافته‌ی خودایفاد شامل ساختار و فرایند اجرای برنامه بود. برای تحلیل داده‌ها از روش‌های آمار توصیفی و استنباطی شامل آزمون‌های ضریب هم‌بستگی پیرسون، تی مستقل و آنالیز واریانس یک‌طرفه استفاده شد. یافته‌ها: نمره‌ی کلی پرسش‌نامه برابر با 25/56 ± 53/332 بود که کمی بالاتر از حد متوسط ارزیابی شده است. بیشترین میانگین نمره در بین ابعاد مربوط به حیطه‌ی فرایند برابر با 85/0 ± 99/4 بود. بالاترین نمره در حیطه‌‌ی ساختار مربوط به طراحی و تدوین پرونده‌ی الکترونیک سلامت، در حیطه‌ی فرایند مربوط به اجرای خدمات پزشک خانواده در قالب پرونده‌ی الکترونیک سلامت و در حیطه‌ی نتایج مربوط به میزان دانش تیم سلامت بود. نتیجه‌گیری: اجرای برنامه‌ی پزشک خانواده در غرب استان گلستان در کل و به‌تفکیک حیطه‌های ساختار و فرایند کمی بالاتر از حد متوسط بود. به‌منظور بهبود وضعیت موجود، لازم است مشکلات مالی اجرای برنامه رفع شود و پیاده‌سازی کامل نظام ارجاع در اولویت برنامه‌های مدیران سلامت قرار گیرد. Introduction and purpose: The goals of the family physician program include creating a referral system in the country, increasing accountability in the health market, increasing people's access to health services, reducing unnecessary costs in the health market, and increasing service coverage. This study was conducted with the aim of evaluating the implementation of the family medicine program from the point of view of managers and staff experts with a process approach in 2022. Methods: In a descriptive-analytical study, 231 heads of health centers of the city and deputies, heads of technical and administrative units (managers) and experts of technical and administrative units (staff experts) in the west of Golestan province participated by census method. The data collection tool was the valid and reliable questionnaire of Abedi et al. In order to analyze the data, descriptive statistics and inferential statistics methods such as Pearson's correlation coefficient, independent t test and one-way analysis of variance were used in SPSS-23 software. Results: The overall score of the questionnaire was 332.53 ± 56.25, which is slightly higher than the average. The highest average score among the dimensions related to the process area was 4.99 ± 0.85. The highest scores were in the area of the structure related to the design and compilation of the electronic health record, in the area of the process related to the implementation of family doctor services in the form of the electronic health record, and in the area of results related to the level of knowledge of the health team. Conclusion: The implementation process of the family physician program in the west of Golestan province was evaluated slightly higher than the average level in general and separately in terms of structure, process and results.
