fa بررسی تأثیر طرح تحول نظام سلامت بر میزان سزارین در بیمارستانهای تحت پوشش دانشکده علوم پزشکی آبادان Evaluation of the Effect of Health Sector Evolution Plan on the Rate of Cesarean Sections in Hospitals Affiliated to Abadan School of Medical Sciences مدیریت خدمات بهداشتی و درمانی Management of Health Services پژوهشي-کامل Research(Original) مقدمه و هدف: یکی از اهداف طرح تحول نظام سلامت، کاهش میزان سزارین و افزایش میزان زایمان طبیعی می‌باشد. در این راستا، پژوهش حاضر با هدف بررسی میزان سزارین‌های قبل و بعد از اجرای طرح تحول نظام سلامت در بیمارستان‌های تحت پوشش دانشکده علوم پزشکی آبادان انجام شد. روش کار: جامعه آماری پژوهش توصیفی- تحلیلی حاضر را کلیه بیمارستان‌های تحت پوشش دانشکده علوم پزشکی آبادان کـه بخـش زایشـــگاه داشـــتند، تشکیل دادند. نمونه‌ها به روش سرشماری وارد مطالعه شدند. بـرای محاسـبه میـزان سـزارین پایه در ابتدا و طی پنج سـال بعـد از اجـرای طـرح، بـه‌ترتیب میزان سزارین در سال‌های 1392 و 1397 بررسی گردید. بـرای محاسـبه تـأثیر طـرح تحـول نظام سلامت بر میانگین صـورت‌حسـاب هزینـه مادران، میانگین هزینه‌های مادران در یک بیمارستان مورد بررسی قرار گرفت. اطلاعات از طریق نرم‌افزار SPSS18 و با استفاده از آزمون‌های کای دو و تی زوجی تجزیه و تحلیل شدند. یافته‌ها: در مجمـــــوع، میـــــزان ســـــزارین در ســال 1392 بــه‌عنــوان میــزان سزارین پایه معادل 45 درصد بود که این میـزان طـی پنج سـال پس از اجرای طرح با کاهشی 24/44 درصدی به 34 درصد رسیده بود. میانگین مبلغ صـورت‌حسـاب بـه ازای هـر مادر بستری، قبل از اجرای طرح تحول سلامت معادل 103342/175 ریال بوده و پس از اجرای آن به 96689/4347 ریال کاهش یافته است. نتیجه‌گیری: کاهش میزان سزارین در بیمارستان‌های تحت مطالعه به هدف اولیه خود دست یافته است؛ اما همچنان متوسط میزان سزارین در این بیمارستان‌ها نسبت به مقیاس جهانی بالا می‌باشد. Introduction and purpose: One of the health sector evolution plan's goals is to reduce the rate of cesarean section (C-section) and increase the natural childbirth rate. Therefore, this study aimed to evaluate the rate of C-sections before and after the implementation of the health sector evolution plan in the hospitals affiliated to Abadan School of Medical Sciences, Abadan, Iran. Methods: The study population of this descriptive-analytical study included all hospitals with a maternity ward affiliated to Abadan School of Medical Sciences. The samples were selected using a census method. The C-section rates were evaluated in 2013 and 2018 to calculate the baseline C-section rate at the beginning and five-year after the implementation of the plan, respectively. In order to investigate the effect of the health sector evolution plan on the mean cost spent by mothers, it was studied in one hospital. The data were analyzed in SPSS  software (version 18) using the Chi-square test, and Paired t-test. Results: The total C-section rate was 45% as the baseline cesarean rate in 2013, which decreased by 24.44% to 34% during the 5 years after the plan. The mean cost spent by each hospitalized mother was 103342.175 Rials prior to the implementation of the health sector evolution plan which was reduced to 96689.4347 Rials after the implementation of the plan. Conclusion: According to the results, there was a reduction in the rate of C-sections after the implementation of the health sector evolution plan in hospitals under study.
